சுகாதாரத்துறை தனியார்மயம் - தாய்மையை கருவறுக்கும் அவலம்

தனியார் மருத்துவமனைகளுக்குச் சென்றால் முதலில் கேட்க்கப்படும் கேள்வி மருத்துவக் காப்பீடு இருக்கிறதா என்பதுதான். ஆம் என்று சொன்னால் ஆபத்து அங்கிருந்து ஆரம்பிக்கிறது என்று பொருள். நமக்கு தேவையோ இல்லையோ எதையாவது அறுத்துக் கட்டி, எக்ஸ்ட்ரா பணம் சேர்த்து காப்பீட்டின் மூலம் கறந்துவிடுவார்கள் தனியார் மருத்துவமனைகள்.

இந்த அவலம் எந்தளவுக்குச் சென்றுள்ளது என்றால், அரசு மருத்துவ காப்பீடு திட்டத்தின் கீழ் தனியார் மருத்துவமனைகளில் சிகிச்சை எடுத்துக் கொண்ட பெண்களுக்கு அவர்களின் கருப்பை வெட்டி எடுக்கப்பட்டுள்ளது. இப்படி வெட்டி எடுப்பது தேவையே இல்லை என்ற போதிலும் பண வெறி பிடித்தழையும் தனியார் மருத்துவமனைகள் இவ்வாறு செய்து வருவது வெளி வந்துள்ளது.

ஒரு பக்கம் தரகு பன்னாட்டு முதலாளிகளுக்கு வருடா வருடம் வரி தள்ளுபடியாக மட்டும் பல லட்சம் கோடிகளில் கொடுத்துள்ள அரசு, இன்னொரு பக்கம் அடிப்படை வசதிகளை தனியார்மயப்படுத்தி வருகிறது. இவ்வாறு மிகப் பெரிய அளவில் தனியார்மயமான ஒரு துறைதான் மருத்துவத் துறை. ஏற்கனவே, ஏழை எளிய மக்களிடம் மதுரை, சென்னை, கோவை நகரங்களில் கிட்னி திருடப்பட்டது போன வருடம் வெளிவந்தது. இந்த வருடம் தாய்மை திருடப்படுகிறது.

மக்களின் வரிப்பணத்தில் தரகு பன்னாட்டு முதலாளிகளுக்கு அள்ளி வழங்கும் அரசு. மக்களுக்கு அத்தியாவசியமான சுகாதரத்துறையிலும் அவர்களை கை விட்டு அனாதரவாக அலைய விடுகிறது. தனியார் மருத்துவமனைகள் ஒன்றுதான் ஒரே கதி என்ற நிலைக்கு தள்ளியுள்ளது. இதனை வைத்து தனியார் மருத்துவ கம்பெனி பிசினஸ் சக்கை போடு போடுகிறது. மக்களின் வாழ்க்கையோடு விளையாடும் மருத்துவத்துறை ஏற்கனவே இந்தியாவில் மிக அதிகப்படியாகவே தனியார்மயப்படுத்தப்பட்டுள்ளது. அதில் மிச்ச சொச்சம் இருக்கும் தொழில் நேர்மை, மருத்துவரின் புனிதம் போன்ற பழைய உலகத்து அடையாளங்கள் சுத்தமாக வழித்தெடுக்கப்பட்டு உலகமய கொள்கைகளுக்கு ஏற்ப மருத்துவத்துறையும் முதலாளித்துவ லாப வெறிக்கு இரையாகி வருகிறது. மெரினா பீச்சில் கிரிக்கெட் விளையாண்டால் மண்டை உடையும், அமைதி கெடும் எனவே தடை செய்தது சரி என்று பேசிய மிக நல்லவர்கள் எல்லாம் இது போன்ற பிரச்சினைகளைப் பற்றி பேசும் போது காணாமல் போய் விடுகிறார்கள்.

நாட்டை வல்லரசாக்குகிறோம் என்ற பெயரில் தனியார்மயம், தாராளமயம், உலகமய கொள்கைகள் மூலம் இந்த நாடு கருவறுக்கப்படுவதன் அம்சங்களாகத்தான் இவை நடைபெறுகின்றன. இத்தகைய தனியார்மயமும், லாப வெறியும், போட்டியும் தொடர்ந்து அனுமதிக்கப்பட்டால் நாளைய உலகம் சுடுகாட்டில்தான் வசிக்கும்.

Hysterectomy On Many Women Was ‘Needless’

Chennai: At 25, it was a surgery Meena Kumari least expected. She had complaints of an irregular menstrual cycle last year. A city-based private hospital then diagnosed a small fibroid as the problem and removed her uterus. Covered as she was by the government employees’ health insurance scheme, the hospital sent a Rs 40,000 bill to the insurance company.

Sounds routine, but her surgery has now been termed “needless” by an expert panel of doctors from the insurance company running the scheme. What’s more, an ongoing audit by doctors working for Star Health and Allied Insurance has revealed that Meena’s case resembled a sizeable number of hysterectomies done under the scheme in the last one year — they were unwarranted and left many young women unfit to conceive.

The findings suggest medical ethics violations and an excessive tendency to use elaborate procedures on patients, presumably to inflate bills. “Many young women need not have gone through the surgery. Many of them needed just medicines to regularise their periods. They had no family history of cancer or any problem with their reproductive system,” said V Jagannathan, chairman, Star Health, who has recently been holding marathon meetings against such practices.

The insurance scheme covers 12 lakh government employees and their dependents. Since its launch in late 2008, 16,000 women have undergone hysterectomies or overectomies for removal of uterus or ovaries. According to the data, 30% of the surgeries were suspect, and of these, at least 540 women were in the age group of 25-35 — and more than 100 in the 20-30 age group. As a result of the surgery, many of those under 30 may have hit menopause early, apart from losing all chances of bearing a child.

Chennai: An audit by doctors working for Star Health and Allied Insurance has revealed that tendency to use elaborate procedures on patients to inflate bills is commonplace.

“One patient told us how she had agreed to the surgery only because she was told the fibroid could turn cancerous,” Star Health chairman Jaganathan said.

Parallel audits on other insurance schemes show that uterus removal for women below 30 years was very low. “We have 1.5 lakh women under the age of 35 covered under the company’s own general health insurance scheme. Of this, just seven women below the age of 35 have had their uteruses removed,” Jagannathan said.

Case sheets of patients billed to Star show that for fibroids as small as 1.4cm, many patients in the government scheme had had their uterus and ovaries removed. Reacting to the findings, fertility expert Dr Priya Selvaraj said: “As far as possible, we ensure that a woman retains her reproductive organs.”

Health secretary VK Subburaj said the health department was yet to get any formal complaints. “If any hospital is found violating rules we will initiate criminal action,” he said. Recently, the insurance company removed two private hospitals in Nagercoil and Kanyakumari for charging extra from beneficiaries of the Kalaignar health insurance scheme.

There are also false claims by some hospitals. A patient who underwent cataract surgery was found not to have an intra-ocular lens implanted as mentioned in the bill.

Finance secretary G Gnanadesikan said state employees had benefited much from the scheme. “We paid premium of Rs 120 crore for 12 lakh employees, who get Rs 2 lakh cover for two years. At the end of the first year, we have received claims for more than Rs 125 crore.”
